Welcome to our Care Coordinators

The WVHII is pleased to announce an exciting, new program in pediatric care called care coordination. Patient care coordination is a patient support service in which trained people guide patients through the healthcare system. Often, patients face gaps in service as well as fragmented care. The patient care coordinator assists to get rid of those barriers to help patients get the care they need. The program makes sure that all patients receive information and assistance to help them obtain:
  • Follow-up care
  • Timely diagnosis
  • Treatment (if necessary)
This will make for better outcomes and greater patient satisfaction. See the profiles of the Care Coordinators on the right side of the page. 
Tri-state Children's Health Improvement Consortium
The Tri-State Children’s Health Improvement Consortium (T-CHIC) is an exciting national collaborative effort of Alaska, Oregon, and West Virginia that is aimed at establishing and evaluating a national system for children's health care quality. T-CHIC is funded by the Children’s Health Insurance Program Reauthorization ACT (CHIPRA) and builds on existing state and practice level change efforts.
The Consortium will provide verifiable documentation of the use of the CMS Initial Core Set of Measures, document and quantify the use of health information technology, and document the effectiveness of using Learning Collaboratives to inform process improvement and identify best practices at a clinic level when becoming a medical home to improve the delivery of children’s health care services in the Medicaid and CHIP populations.
There are 10 WV practices participating in the project. Below are the Care Coordinators who are helping with this project.

Sherry Dasher
Sherry Dasher
Sherry is a native West Virginian. She graduated from James Madison University in 1981 with a BA degree in Political Science. She has spent most of her career working in social services and community development. In her spare time, she enjoys creative writing, reading, outdoor activities,and pursuing the virtues of a rural lifestyle. She lives in Greenbrier County with her husband and their various pets.

Angela Gibson
Angela started in the health field more than 12 years ago. She obtained her Phlebotomy license in 1997. During her phlebotomy training she learned about the different diseases and health issues that people face daily. After completing her phlebotomy classes she realized she really enjoyed the health field and wanted to be a part of improving people’s health. Her interests have driven her throughout the years to learn as much as she could about improving people’s health. For almost seven years she worked for a wellness company who promoted health and wellness for employers and their employees. She is very excited and looking forward to working with others to be a part of improving children’s health and wellness.
Brittany Halstead
Brittany Halstead
Brittany is the new Patient Care Coordinator for the Children's Medicine Center, located inside CAMC Women and Children's Hospital. She has been a Medical Assistant for seven years and has an Associate's Degree in Allied Health and Human Services. She has three children and spend any extra time she has with family and friends. She looks forward working with the health care team to provide the best care possible for your family.
Julie Justice
Julie Justice
Julie proudly assists and advocates for patients at Marshall University Pediatrics in Putnam County, WV as a Care Coordinator for the West Virginia Health Improvement Institute. Julie has a background in the social work field, although her education derived from WVU Tech, as a Business Management Major with Sociology and Marketing Minors. She was on the Dean’s List and involved in sports. Julie was active in clubs, such as the Society of Human Resource Management as a Publicity Officer and a member of the Student Government Association. Julie was previously a Case Manager and Team Leader for an aftercare program that served chronically homeless individuals with co-occurring disorders of mental health and/or substance abuse issues. She graciously accepted the Leon Sullivan Award of Excellence and Equality, that was voted on by her employer and peers. She volunteers some of her time to continue serving the homeless, as well as participating in events for the Alzheimer’s Association, March of Dimes for Babies, and donating to Locks of Love.

Lydia Ours
Lydia Ours is the Care Coordinator at Tri-County Health Clinic, in Rock Cave. She enjoys working with children and youth and has a strong desire to make a difference in her community. She received her Bachelor of Arts degree in Elementary Education from WV Wesleyan College; she has worked as a preschool teacher and currently also serves as a Children’s Pastor at Cornerstone Community Church. Lydia resides in Buckhannon, WV with her husband, Dustin, and her cat, Max.
Dawn Persinger
Dawn Persinger
Dawn is a graduate of Allegheny College of Maryland, Suma Cum Laude. She has been married for 25 years to William Persinger, Cpt. WV Department Natural Resources Cpt. USANG 130th Reserve Unit, and has two sons. Zachary married to Jessica, residing in Summersville with two children. Wesley married to Jillian, residing in Fairmont. Dawn is a member of New Life Assembly of God and is a lover of life, enriched by girlfriends, laughter and an ever growing cute shoe collection. Crazy mad about knitting and a proud new owner of a white boxer pup named Daisy.
Kimberly Skidmore
Kimberly Skidmore
Kimberly is from Harpers Ferry West Virginia. She recently graduated from West Virginia University with a degree in family studies and child development. During her free time Kimberly likes to read and visit with her friends and family. She loves WVU football games and traveling to the beach.
Robin Spence
Robin Spence
Robin Spence, of Lewisburg, comes to the WV Health Improvement Institute with an interest in prevention. With a strong background in community development she is currently working on her Masters in Health Promotion. In her spare time, Robin likes to garden, travel and do ballroom dancing.
Brook Taylor
Brook Taylor
Brook graduated West Virginia University Institute of Technology in 2005 with a Bachelor of Science in Nursing. As an RN, she has come to love community nursing, and is anxious to take her role as patient advocate to its highest level. She and her husband Jamie are both natives of Clay County and are proud to call West Virginia home. Her daughter Sophie and son Spencer are her greatest blessings. Like much of Appalachia, they enjoy family life and close community relations. She is extremely excited to work within in her community to assist in the coordination of care on a holistic level. Her goal is to serve her neighbors in building a healthier community for themselves and children.

Sandra Vanscoy
Sandra has been married for 24 years, and has 2 sons, Travis (23) and Trevor (18). Family is most important to her and she cherishes every moment they have together. Sandra is a nurse by profession as well as by nature. Her nursing knowledge will serve participants well during this project and she is happy to have the opportunity to continue to be part of the coordinated care of the patients she had been caring for previously.
