Innovation Community Application
The West Virginia Health Improvement Institute would like to invite you to become a participant in the Institute’s Innovation Community and an early adopter of the medical home model of primary care. The Innovation Community is a virtual community that allows for primary care practices within West Virginia to receive information, technical assistance and training on the medical home model as part of the CMS Medicaid Transformation grants awarded to the State’s Medicaid program. It is an environment where practices will be able to collaborate with each other in order to accelerate adoption of the medical home model and to improve the quality of care delivered.
The Innovation Community is also the laboratory where Transformation Grant resources and contributions from supporting sponsors (e.g. the payer community in West Virginia) will be channeled to conduct pilots and demonstration programs on cutting edge issues such as e-prescribing; electronic health records; new reimbursement models for primary care; emerging models to enhance patient self management; and innovations in improving patient care. By participating in the Innovation Community you will help shape changes in primary care delivery and lead the transformation of health care in West Virginia.
