Access and Communication is the topic of the first standard for PCMH recognition, but it’s not only about recognition as a medical home. Eileen Patterson lead the last call in this series on Friday, October 23, 2009, 12-1 PM ET.

Continuity and Teams
.”
Continuity is ”king.” It is a basic premise of population management and enables patient-centered care and patient self-management. Continuity increases efficiency, quality of care and patient, staff, and provider satisfaction. It decreases errors, promotes safety, and decreases rework. In other words, continuity is associated with a variety of factors that lead to a greater quality of patient care and patient quality of life. But continuity of care is not just a provider issue, it is a team issue. Being able to function as an effective team is also important to providing continuity.
