WVHII Clinical Process and Outcomes Measures


The Measurement, Reporting, and Reimbursement Work Group has endorsed a set of clinical process and outcomes measures that can be used for tracking overall progress toward improvement of health status and outcomes in the state. The data can also be used by each practice to monitor their internal improvements in care delivery and to drive change in the measures over time.

 Diabetes  National reference
 Percent of patients 18-75 with type 1 and type 2 diabetes who have had A1c test in last 12 months  HEDIS measure available
 through claims
 Percent of patients 18-75 with type 1 and type 2 diabetes whose most recent A1c in the last 12 months is > 9  HEDIS, HRSA, and PQRI measure
 Percent of patients 18-75 with type 1 and type 2 diabetes with retinal or dilated eye exam in last 12 months  HEDIS and PQRI
 Percent of patients 18-75 with type 1 and type 2 diabetes with LDL exam in last 12 months and a value of < 100   HEDIS and PQRI
 Percent of patients 18-75 with type 1 and type 2 diabetes with Blood Pressure < 140/90 in the last 12 months

 Percent of patients 50 years of age or older with an Influenza vaccine within the last 12 months  HEDIS, HRSA and PQRI
 Percent of patients 65 years of age or older with a Pneumococcal Vaccination

 Percent of women 40-69 who have had a mammogram in the last 2 years  HEDIS, HRSA and PQRI

 Percent of patients 50-80 who have met screening criteria for Colorectal Cancer
 • Colonoscopy within last 10 years
 • Double Contrast Barium Enema within last 5 years
 • FOBT within the last 12 months
 • Flexible Sigmoidoscopy within last 5 years

 a prescription for long-term control of their asthma
