The West Virginia Health Improvement Institute has established an Innovation Community to facilitate provider engagement in accelerating Medicaid Transformation and the migration to the medical home model of care delivery. Through the Innovation Community, the Institute will assist health care providers in their preparations for qualification as a “Patient-Centered Medical Home”. Participants in the Innovation Community will also be eligible to participate in pilots and demonstration programs that are part of the work of the Institute.

Below are descriptions of some demonstration pilots that are now being tested in the Innovation Community. There are many more pilots being launched. For more information click here.
This pilot is being sponsored by the Measurement/Reporting/Reimbursement Work Group. The aim of this pilot is to promote BMI screening at all primary care visits for children and to introduce a standard set of interventions for all children with BMI at the 85th percentile or above. The pilot will be conducted over a two year period in 10 practices throughout the state.
Q1 update: Baseline data has been collected by each of the practices and the intervention period is now underway.
This demonstration project is being sponsored by the West Virginia Health Care Authority. The aim of the demonstration is to eliminate elective deliveries at less than 39 weeks of gestational age. The demonstration will involve multidisciplinary teams from ten communities in the state and will focus on strategies that can be spread throughout the state.
Q1 update: The first learning session for the 14 participating hospital teams was held in late January and each team is now working on data collection and interventions to address their pre-term deliveries.
